Home Support Assessment

What is a home support assessment?

A qualified Home Support Assessor will visit with you in your home to talk to you about what support you need. They can give you information on what is available across the Peninsula to support you to continue to live independently, safely and actively in your home. 

You can have a family member, interpreter or advocate with you during the assessment. If you would like help to find an advocate please let us know and we will be happy to help you with this.

How do we cater for diversity?

Our Home Support Assessors work in an inclusive manner, treating each person as an individual, with sensitivity to your needs, beliefs and values. 

At your request, or if it becomes apparent communication would be improved if an interpreter was present, we will arrange for an accredited interpreter to be present. 

Some of our written materials are available in languages other than English.
