| Access to Aged Care Services | | Referrals for services are accepted by the Shire directly from residents, families or friends, doctors, hospitals and other community based agencies.
Prior to receiving a service, a Shire officer will discuss and assess the type, level and regularity of services required by a resident. One or more Shire services may commence following this assessment and/or the Shire officer may also make referrals to other specialist agencies on the resident's behalf if required. Services have sliding fee scales that are based on household income and each individual's capacity to pay.
All recipients of Home and Community Care services are reviewed regularly as determined at the original assessment, or at the request of an individual or family using services. The review process provides a way of identifying changing needs in a family/individual's circumstances and ensuring the services are able to respond appropriately.
The changing needs of people using Home and Community Care services are documented and used to inform and plan the further development of services for clients.
The review process is also an opportunity for Shire staff to update users on any changes to the existing service and to receive feedback on any issues or concerns users may have with regard to the way the service is provided.

| General Home Care | | The General Home Care Service provides a range of practical assistance to support residents in the day-to-day running of the household. The service aims to support the resident's independence by undertaking tasks that the resident can no longer do themselves.
The service is available Monday to Friday from 7.00am to 7.00pm. Service outside these hours may be arranged, subject to assessment.

| Home Maintenance | | The Home Maintenance service provides assistance with a range of tasks to ensure resident safety and security. The service aims to assist people in maintaining a safe and secure home environment by undertaking minor repairs to the resident's home, furniture or equipment. The service is available Monday to Friday between the hours of 8.30am to 5.00pm.

| The service does not undertake cosmetic maintenance or work which requires a qualified tradesperson but does provide clients with advice regarding requirements for tradespeople.

| Personal Care | | The Personal Care Service assists clients with their personal and hygiene needs. Staff are experienced and qualified in the provision of personal care and receive on-going training through the Shire’s educational programs. The service is available Monday to Friday from 7.00am to 7.00pm. Service outside these hours may be arranged, subject to assessment.
The service also undertakes other personal care tasks which do not required the skills of a trained nurse.

| Respite Care | | The Respite Care Service is available to carers of older people and people of any age with a disability, including young children with a physical, intellectual or sensory disability.
The service assists carers to participate in family, social, recreational, personal and community activities. Personal Care assistance is also available during a respite care visit.
Staff are experienced and qualified in the provision of respite care activities and receive on-going training through the Shire’s educational programs. The service is based on assessed need and aims to be flexible to support carers. Overnight visits may also be available in some cases.

| Delivered Meals | | The Delivered Meals Service home delivers a nutritious meal to residents who are unable to provide for their own personal dietary needs.
Meals consist of soup, main meal and dessert. A vitamin C supplement is also provided in the form of fruit juice. The service is able to cater for a range of special dietary requirements including diabetic, vegetarian, low fat, cut-up and vitamised meals.
Meals are delivered Monday to Friday from 9.45am to 1.00pm, depending on the location of the delivery. If required meals can be provided for weekends and public holidays.

| Adult Day Activity Support Service | | The Adult Day Activity Support Service operates a centre-based program on Mondays at the Somerville Senior Citizens Centre and a community access program on Thursdays and Fridays in the Hastings/Mornington areas. It provides a program of activities for residents with a physical, intellectual, cognitive or sensory disability. It also provides respite for the regular carer.
A range of indoor and outdoor activities are designed to suit the needs of individual participants to increase their opportunity to participate in community life and enhance their wellbeing.
A bus service is available to collect and drop off participants to and from their home. The bus is fitted with a wheelchair hoist to transport clients if required.

| Metro Access | | MetroAccess is a key initiative of the Victorian State Disability Plan 2002-2012 that aims to help local communities to become more accessible and inclusive of people with disabilities.
The initiative aims to provide a comprehensive response to the key issues experienced by people with disabilities in local communities. Based on the principles of community building, MetroAccess works to achieve long-term sustainable change in local communities, bringing together the strengths, resources and creativity of individuals, communities and government to enhance the community membership and participation opportunities for people with disabilities.
A project officer works across all areas of community infrastructure such as health, housing, education, employment, transport, community services and leisure, to plan and develop opportunities for greater participation and inclusion.

| All Access Programs | | The Shire is committed to fostering a more socially connected community by engaging older residents, people with a disability and those of us who are usually inactive to participate in leisure and recreational activities available on the Mornington Peninsula.
The Shire runs several ‘All Access’ programs around the peninsula covering activities including Wheelchair Golf, Discos, exercise for older people and those with a disability.

| Disabled Parking Permits | | A State-wide Disabled Persons’ Parking Scheme currently operates in Victoria. The Scheme provides for two permit categories with varying parking concessions, based on the applicant’s need for assistance. Under Category One, permit holders with significant intellectual or ambulatory disabilities who meet the eligibility criteria are entitled to park a vehicle in a special bay reserved for people with a disability only, for the specified time only, or may park a vehicle in any ordinary area or bay for twice the specified time (upon payment of any initial parking fee, if applicable). Under Category Two, permit holders who require rest breaks when walking may park a vehicle in any ordinary area or bay for twice the specified time (upon payment of any initial parking fee, if applicable). The permit label should be carried by the permit holder when he/she is not being transported in a vehicle. Category two permit holders are not permitted to park in special disabled persons’ parking bays.
Permits must not be left on display when the vehicle is not engaged in the process of transporting a permit holder.

| Disability Access - have your say | | The Mornington Peninsula Shire (MPS) and the MPS Disability Consultative Committee are seeking your views on disability access on the Peninsula. The Shire has appointed Access Audits Australia (AAA) to facilitate a review of the Shire’s Access (Disability) Plan.
What is an Access (Disability) Plan? The Disability Action Plan guides all access planning within the Shire and is a systematic approach to improving disability access for the communities that live in, or visit, the Mornington Peninsula. You can download a copy of the current plan from the bottom of this webpage.
How can I have a say? The aim of the review is to enable you to provide suggestions and information to guide further actions and increase access to the Shire’s physical and social infrastructure and services. You can contribute either by filling the online survey form at the bottom of this webpage, by attending the Community Access Forums on either the 7th or 12th August (visit our Events Calendar for details), or both.
